If you are unable to fill out the form online, you may receive a phone call or a visit from Health Centre staff to collect this information. This process is for on-reserve households only.
Emergencies can happen at any time. This includes flooding that affects homes or roads, evacuations, fires, severe weather like tornadoes, long power outages, and environmental emergencies. By completing the form, you help us identify vulnerable households and make sure no one is missed when help is needed.
Your information will be kept private and safe. All information will be stored and handled under PHIPA guidelines and only trained Health Centre and Emergency Response staff will have access.
