The Home and Community Care (HCC) program supports clients live independently in the community by offering a continuum of home care services. Registered Nurses, Certified Personal Support Workers and Homemakers provide various services such as: case management, referrals and linkages; client assessment; home care nursing; home support services; respite services; transportation; and a medical equipment loan program.
The HCC supplements but does not replace our clients’ efforts to care for themselves with the assistance of their family, friends and the community. We help those who want to keep their independence in their own home and to be close to their loved ones as long as possible.
Community Support Services
Diners’ Club (Congregate Dining)
With the support of a dedicated group of community volunteers the HCC and Healthy Living programs offer a Diner’s Club program. You do not need to be an HCC client to enjoy a nutritious meal and socialize in a friendly setting. The Diner’s Club is open to those aged 55 and up. Transportation is available.
Help Line
This personal response service ensures that clients living on their own can get quick assistance—24 hours a day. The client just presses the help button (pendant or wristband) and a certified Helpline monitor will respond, assess the situation and contact the appropriate help, whether it be a neighbour, relative or ambulance.
Home Support Services
Our certified Personal Support Workers (PSWs) are trained in healthy meal preparation, bathing, monitoring medications and other personal care. Our PSWs and Homemakers also provide homemaking duties for those who are not able to do so on their own safely.
Our HCC Nurses provide assessments, which identify what is needed in a care plan in order for us to best meet the medical and health needs of our clients.
Medical Equipment Loan Cupboard
This service provides a wide range of medical equipment such as walkers, canes, hand rails, commodes, and hospital beds that are available for loan on a short-term basis.
Respite
Live-in caregivers that need to attend appointments, work or their own personal care may request respite services. This ensures the HCC client is not left alone and provides some down time for the caregivers.
Seasonal Services
Assistance with seasonal help around the home for HCC clients who are unable to safely do it themselves is available. This includes snow removal, grass cutting, and seasonal housing cleaning.
Security Checks
Some clients living alone or home alone after a change in medical condition may benefit with a check in from a PSW. The HCC program offers scheduled security checks in person or by phone.
Transportation Services
Non-medical transportation services are available to HCC clients to run errands. We offer transportation 3 days a week, including scheduled trips to North Bay and Sturgeon Falls. Clients are required to book trips 24 hours in advance.
Assisted Living
Assisted living services are intended to address the needs of high risk seniors so they are able to remain safely in their homes while reducing or avoiding unnecessary hospitalizations, emergency room visits and admission to Long Term Care homes.
Assisted Living Services provide senior clients with a full range of personal support services. These services are provide at the required times as needed over a 24-hour basis. This helps seniors who need daily personal care, meal preparation, and security checks to continue to reside at home safely and comfortably. These seniors must be able to safely remain in their homes alone between visits.
